What is Active Travel
Social Prescribing?
Have you ever wondered how much your community could shape your health and well-being? Social prescribing, or community referral, is a simple yet powerful way for GPs, nurses, and other health professionals to connect people with local services that support their overall well-being. But what if your journey to better health could include walking, cycling, or wheeling with others right in your neighbourhood?
With Active Travel Social Prescribing, we do just that—helping you access fun, supportive activities that fit into your everyday life. We know that your health isn’t just about doctor visits—it’s shaped by your environment, your community, and your daily habits. That’s why we’re bringing physical activity to your doorstep, making it easier for you to take control of your health and develop lasting, positive changes.
Are you ready to start a new journey toward healthier habits?
