What is Health and Wellbeing Coaching?

Health and Wellbeing Coaches are trained professionals who partner with you to improve your life. Using behaviour change techniques, they help you set and achieve goals related to your health, long-term conditions, or mental wellbeing. Whether it’s managing stress, coping with a health condition, or making lifestyle changes, our coaches offer encouragement and support tailored to what matters most to you.

Training With our nice coaches!

Our coaches work with you one-on-one for up to 6 sessions, either in person, over the phone, or via video call. They can assist with:

  • Improving sleep quality
  • Managing everyday mental health challenges like stress, worries, or low mood
  • Applying lifestyle changes recommended for long-term conditions (e.g., diabetes, hypertension)
  • Building confidence to take control of your wellbeing

Together, you’ll create a plan that’s meaningful to you, with the coach providing the tools and motivation to succeed.

Who Can Access This Service?

Eligibility

This service is available to anyone over 18 who is registered with a GP practice in the North Solihull Primary Care Network. If you’re unsure whether your GP is part of our network, contact your practice or give us a call.

How to Get Started

Simply speak to your GP or another health professional at your practice about being referred to a Health and Wellbeing Coach. They’ll help you take the first step toward better health and wellbeing.

How It Works

1. Referral

Talk to your GP or health professional to get referred to our service.

2. First Contact

A coach will reach out to discuss your needs and schedule your first session.

3. Personalised Plan

Over up to 6 sessions, you’ll work together to set goals and build a plan that suits you. Sessions can be face-to-face, by phone, or online—whatever works best for you.

4. Support & Progress

Your coach will provide encouragement, resources, and strategies to help you succeed, adjusting the plan as needed.

5. Moving Forward

After your sessions, you’ll have the confidence and skills to maintain your improvements independently.
